The Verity is less than 3000Grt and does not need to carry a VDR or a ECDIS (Electronic chart). At 04:00 there is normally a watch change but the Verity may only have two navigation officers (captain and one mate) so the mate onboard was probably on watch at the time. That is also the time for the least experienced officer to have the watch.
As I have been piloting vessels of the size of Verity for 25 years I know that the lookout is often missing on the bridge. Especially after a day loading where the deck crew has been very busy.

Some older captains also have a standing order for the mate(s) to not slow the speed (many times from old habit or that they have a generator on the propeller shaft and need constant revolution on the propeller).

This may be what we call a "radar assisted" collision, see the collision between Stockholm and Andrea Doria.

I speak as an amateur yachtsman rather than a professional seafarer, but my yacht did have both radar and AIS. It was customary to listen out on VHF channel 16 and if I was in any doubt as to the intention of other vessels or wished to inform them of mine, I would give them a call. Is this not the practice on commercial vessels?

bad seamanship, there was no watch keeping in both Bridges which resulted to five dead Seaman's, sickening……..

Depending on visibility, navigation lights should be clearly visible, visual bearings are a good indication of risk of collision

Polesi appears to have evaded to port. She should not have done so. You mention that Polesi might have considered Iona as a factor. Possibly, but Polesi should not have done so: Under no circumstance would Iona have had an overtake right of way. If, however, Iona had drifted beyond 22 degrees off Polesi’s centerline, then Polesi MIGHT have thought that Iona was attempting an overtake at distance. Problem is, Iona was on Polesi’s port, which is yet another reason why Polesi should not have attempted evasion to port. Am I wrong?
