  1. My mother always said it takes two to make an accident. Verity should have slowed, Polesie misjudged Verity speed, and should have gone to starboard to avoid both ships.

  2. If Verity had slowed and turned to starboard to avoid Polesie, it would have put her in collision with the cruise ship. A difficult situation for all concerned. Let’s hope the fatalities remain at one.

It looks as if both ships on seeing each other at the last moment both took avoiding action the Polesie turned to port as if to cross astern of Verity and at the same time Verity turned to starboard to pass astern of but unfortunately made the situation worse by turning into each others path neither ship anticipating the others intention both ships expecting he other to hold their course .
    One can only feel for the crews involved and for the lost crew members and their family's at these times..

  5. In regard to the yacht sinking in Mexico,I always wondered about those large port lights in the hull and those were way down by the water line. Shaky business.

  6. Even U.S. warships who may have the world’s best radar and other sensors have collisions at sea. It is always human error in the long run. All the sensors in the world won’t prevent a collision if someone forgets to turn them on or mutes an alarm.

  7. Fatigue is never an excuse. Everybody on land, sea, or air is always at a state of fatigue. That is the curse of the modern world. They knew their job, the failed to do their job.

  8. All multi-million dollar machines, whether it be 20 million or 200 million, are "bugged" by manufacturers. They keep an eye on their expensive stuff, all the time, in real time.

  9. Just wondering the distance between Polesie and Iona. Verity may have been wondering about giving way to Polesie would also mean having to wait or give way to Iona. Was there enough distance for her to slip in between the two ships. Perhaps the options were to come to an almost dead stop or turn 90 starboard or port and let both ships pass. But port side was busy with traffic heading south. Difficult to tell without the ships speeds and the distances between them along with their stopping distances

  10. Rule 17 c indicates that the stand-on vessel (in making avoidance when the give way does not) should not turn to port. Turning towards is almost always a bad move. Also if both ships turn to starboard (right) then CPA increases.

  11. Looking at the AIS at based on the cruise ship had the Polesie stayed on a constant bearing and not turned it looks as though the chance of passing by would have been greater

  12. When I did my masters ticket we all understood that NEVER turn to port to avoid a collision. The give way vessel can slow down or failing that turn to starboard and go behind the stand on vessel. The stand on vessel should not turn to port as it then blocks the give way vessels second opportunity to avoid the collision. it seems via the AIS that Verity was turning starboard and the other vessel turned port. only result can be a collision. As you said, AIS can not be used to verify the last movements of the vessels. STill, it seems the wrong turn was made.

  13. The channel "Whats going on with shipping", has a good breakdown on the occurrence's before the accident. The Verity should have gave way, based on the AIS data. This was caused by the biggest problem facing the world, people not paying attention, and not communicating with each other. This could have been avoided.

  14. R.I.P the merchant seamen who sadly lost his life, unfortunately.the 4 missing will no doubt have also drowned. Must be horrendous going down in a ship but in the dark even worse

  15. At least it is in shallow water so any necessary retrieval will be easier. I note there are three wrecks in close proximity .It would be helpful to know those wreck dates to determine if this crossing point is a particularly dangerous area for some reason.
    With the cruise ship in close proximity astern of Polise this may have complicated Verity's decision making re turning behind Polise but it does not excuse .

  16. Thank you for the interesting video. Do yachts (at least those above a certain size) normally have Voyage Data Recorders? And who could actually compel the surviving ship to hand over their data? Can German authorities do that or just their flag state (Bahamas)? How would they prevent it from simply recording over the data (given it only records the last 12 hours) just in case someone on board screwed up and they are trying to cover it up?

  17. If Verity has radar and collision warning alarms, the most obvious explanation is the person at the helm was asleep or otherwise incapacitated. It's difficult to understand how they wouldn't have reacted under any other circumstances. Mechanical failure would result in the captain calling out a securité distress warning while they figured it out. Mayday or Pan Pan if they attempted to make a normal course correction action to avoid the conflict and the vessel controls unexpectedly failed to respond.

  18. Amazing we had very few collisions of large maritime vessels in memory and now it is commonplace. This all look nepharious to disrupt global supply chains for 'the great reset'.

  20. Polesi appears to have evaded to port. She should not have done so. You mention that Polesi might have considered Iona as a factor. Possibly, but Polesi should not have done so: Under no circumstance would Iona have had an overtake right of way. If, however, Iona had drifted beyond 22 degrees off Polesi’s centerline, then Polesi MIGHT have thought that Iona was attempting an overtake at distance. Problem is, Iona was on Polesi’s port, which is yet another reason why Polesi should not have attempted evasion to port. Am I wrong?

  21. I speak as an amateur yachtsman rather than a professional seafarer, but my yacht did have both radar and AIS. It was customary to listen out on VHF channel 16 and if I was in any doubt as to the intention of other vessels or wished to inform them of mine, I would give them a call. Is this not the practice on commercial vessels?

  22. The Verity is less than 3000Grt and does not need to carry a VDR or a ECDIS (Electronic chart). At 04:00 there is normally a watch change but the Verity may only have two navigation officers (captain and one mate) so the mate onboard was probably on watch at the time. That is also the time for the least experienced officer to have the watch.
    As I have been piloting vessels of the size of Verity for 25 years I know that the lookout is often missing on the bridge. Especially after a day loading where the deck crew has been very busy.

    Some older captains also have a standing order for the mate(s) to not slow the speed (many times from old habit or that they have a generator on the propeller shaft and need constant revolution on the propeller).

    This may be what we call a "radar assisted" collision, see the collision between Stockholm and Andrea Doria.
